I, ____________________________ , as Student (if over 18) or Parent/Guardian of the above-listed Student(s), do hereby, for myself, my heirs, assigns, and legal representatives, waive, release, and forever discharge any and all rights and claims for damages or injuries which may result while attending inside the premises of Twin Dragons Martial Arts & Black Belt For Life Studios, Inc.

I fully understand and acknowledge that injury or damage—including pain, illness (including COVID-19), paralysis, other personal or psychological harm (temporary or permanent), or damage to personal property—can occur while participating as a student. I accept full responsibility and agree that any costs, expenses, or liabilities resulting from any such occurrence shall not be borne by Twin Dragons Martial Arts & Black Belt For Life Studios, Inc., its officers, owners, employees, instructors, members, or staff.

I further waive, release, and forever discharge Twin Dragons Martial Arts & Black Belt For Life Studios, Inc., its agents, instructors, owners, and members from any and all rights and claims for damages arising from the use of any training or exercising equipment, weapons, or transportation vehicles used in connection with the studio, including any illness, injury, damages, or sickness from any infection during the membership or introductory period.

Furthermore, I recognize Juan Arzani, Rey Castro, and Steven Tarrago as Owners, Directors, and Instructors of Twin Dragons Martial Arts & Black Belt For Life Studios, Inc., and do hereby waive, release, and forever discharge any and all claims for damages, personal or otherwise, against them or their family members.
